This could also indicate bleeding gums, or lacerations in the mouth or throat.
You use the term spitting up. If you mean vomiiting blood, you're discussing a hemorrhage -- and this one requires a 911 call (possibilities include upper or lower gastric hemorrhage, esophageal varices, and other conditions).
If you mean coughing up blood, this could indicate an insult to the lungs -- a visit to the hospital is called for now. If the bleeding is copious, call 911.
